Prequalify
In order to qualify for a medical marijuana card in Arizona, the following must apply.
1. Must have an Arizona driver’s license, Arizona state ID, or current passport
2. Must not be pregnant or Nursing
3. Must not be diagnosed with schitzophrenia
4. Obtain medical records from a treating physician
The certifying physician must have progress notes for a visit within the last 12 months from a treating physician. Download and fill out our medical release form and email or fax it back and we will obtain the records for you. If you have not seen a physician in the last year, you may be able to get an in house examination (some conditions do not apply). Please contact our office to discuss your options.

The Process
1. Check to see if you have a qualifying condition. The following conditions are approved by the state of Arizona for a medical marijuana card.
• Agitation of Alzheimer’s Disease
• Amyotrophic Lateral Sclerosis
• Cancer
• Crohn’s Disease
• Glaucoma
• Hepatitis C
• HIV/AIDS
• PTSD
• Cachexia or Wasting Syndrome
• Severe Nausea
• Severe and Chronic Pain
• Seizures
• Severe and Persistent Muscle Spasms



2. Review of medical records.
Arizona Department of Health requires medical records from within the last 12 months. If you have seen a doctor in the last year, we can obtain the needed records for you. Download and fill out our medical release form and email or fax it back. If you have not seen a physician in the last 12 months, you may be able to get an in house examination. Please contact our office to discuss your options.



3. Certification Appointment and Costs.
You will meet privately with a board certified and experienced physician. Appointment time is approximately 30 minutes. Unlike most clinics we have little or no wait time.

FEES

Doctor’s Visit WITH RECORDS: $95 cash or $110 credit card.

If you have not seen a doctor in the lasts 12 months and need an exam, the doctor’s visit is 115 (cash) or $130 (credit card). In office diagnoses available for some conditions. Please call to verify we are able to diagnose your condition same day.

To process the application on your behalf is $20. In addition to getting certified for your medical marijuana card, other pain management treatment options will be made available to you. Home visits available. PLEASE SEE BELOW FOR AZDHS FEES.



4. Application to Arizona Department of Health.
Once your evaluation is completed, an application needs to be processed with Arizona Department of Health at https://medicalmarijuana.azdhs.gov. AZDHS Fee is $150 or $75 with food assistance for the two year card. The card is electronic and you will receive a registration link within 3-5 business days.
